Accreditation Information

During this live and interactive webinar, social workers, nurses, and board-certified patient advocates in attendance will learn about Social Determinants of Health and how inequities impact an individual's chances of developing cancer or other diseases, how SDOH related to work, transportation, school, environment, and literacy can impact one's health, and things to do to try to minimize it.

This webinar will provide social workers, nurses, and other types of health care professionals with a foundation to help patients understand SDOH related to work, transportation, school, environment, and literacy, and provide appropriate referrals. All attendees will have the opportunity to ask the speakers questions in real time via a chat box.

Nurses

Triage Cancer is accredited as a provider of nursing continuing professional development by the American Nurses Credentialing Center’s Commission on Accreditation. Nurses who meet the requirements will be awarded 1.5 contact hours.

90 minutes of instruction/60 minutes = 1.5 contact hours

Social Workers

Triage Cancer, #1689, is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Organizations, not individual courses, are approved as ACE providers. State and provincial regulatory boards have the final authority to determine whether an individual course may be accepted for continuing education credit. Triage Cancer maintains responsibility for this course. ACE provider approval period: 08/26/2020 to 08/26/2023. Social workers completing this course receive 1.5 continuing education credits.

Triage Cancer is recognized by the New York State Education Department's State Board for Social Work as an approved provider of continuing education for licensed social workers #SW-0626. Social workers completing this course will receive 1.5 contact hours.

This course, Beyond Medicine: Your Community & Your Health, Approval #01012023-1, provided by Triage Cancer is approved for continuing education by the New Jersey Social Work Continuing Education Approval Collaborative, which is administered by NASW-NJ. CE Approval Collaborative Approval Period: Friday, January 6, 2023 through August 31, 2024. New Jersey social workers will receive 1.5 Non Clinical CE credits for participating in this course.

Board Certified Patient Advocates

Triage Cancer (Provider Code BCPA-ACE2006) Webinar “Beyond Medicine: Your Community and Your Health” is approved for 1.5 CEs by the Patient Advocate Certification Board to satisfy the requirements for Board Certified Patient Advocates (BCPA). Program Approval Code BCPA-ACE2006.20230427.

Triage Cancer is recognized by SHRM to offer Professional Development Credits(PDCs) for SHRM-CP® or SHRM-SCP® recertification activities.

Other Professionals

If you would like a general certificate of completion, you must indicate that when completing the webinar evaluation form. You must attend the entire event.

Evaluation forms are emailed after the webinar and are required for a certificate. You will also receive a post-test, which is not mandatory, but strongly encouraged.

  • 2:15pm – 2:20pm Central – Questions
  • 2:20pm – 2:30pm Central – Post Test

Objectives

  • Attendees will be able to define Social Determinants of Health.
  • Attendees will be able to explain how SDOH impact the chances of developing cancer and successful treatment.
  • Attendees will be able to articulate at least one action that can be taken to reduce the negative impact of SDOH in areas like work, school, and access to care.

Requirements to Receive Credit (all professionals)

In order to receive CE/contact hour/PDC, attendees must register individually. Participants who “listen in” on another participant’s phone/computer will not be eligible for credit. Participants must attend the entire webinar. No partial credit will be given.

Post-test & Evaluation

Participants will be emailed a link to the online evaluation and post-test one DAY after the completion of the webinar and will have one week to complete both. Participants must receive a passing grade (80%) on the post-test to receive CE/contact hour (only required for nurses and social workers). Participants who do not pass the post-test will be given one opportunity to re-take the test. Instructions and a link to re-take the exam will be emailed to every participant who does not receive a passing grade on the post-test. If participants do not pass the post-test on the second attempt they will not receive credit.

Certificates

The relevant certificates will be emailed to eligible attendees within 4 weeks of the event.
